Job summary

WelbeHealth seeks Home Care Assistants to deliver compassionate personal care in participants’ homes and support the care plan as part of our IDT. You will provide ADLs assistance, monitor vital signs, and help with home exercise programs after training.

Ideal candidates have a high school diploma, BLS certification, and a CNA/HHA credential preferred. Bilingual abilities are a plus, with a Hospital Registry requirement before starting and CPR/First Aid due within 180 days.

Qualifications

• High school diploma or equivalency.
• Current Basic Life Support (BLS)/First Aid Certification required.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• Bilingual preferred.
• Must be registered with Home Care Registry (HCA) prior to start date.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including bathing, dressing, grooming, toileting, and safe lifting.
• Assist with Instrumental ADLs such as laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments and assist with safe transfers.
• Assist the Rehabilitation team with home exercise programs as indicated in the care plan.
• Provide translation services as needed.

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.

Our Home Care Assistants are a vital part of our team, providing aid to our participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.

This role is different becauseHome Care Assistantsat WelbeHealth:

• Work a stable, full-time schedule – unlike traditional home care roles, you will be assigned participants to fill a full workday, and will be re-assigned in cases of cancellations or unforeseen circumstances
• Visit 2-5 patients per day depending on participant needs and care plans, working shorter individual shifts that still add up to an 8-hour workday
• Receive reimbursement for travel time and mileage between participants

On the day-to-day, you will:

• Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes
• Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed
• Provide translation services as needed

Job requirements include:

• High school diploma or equivalency
• Current Basic Life Support (BLS)/First Aid Certification required
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
• Bilingual preferred
