Job summary

Hackensack Meridian Health is seeking a registered nurse to provide professional home health care. You will assess, plan, implement and evaluate patient needs in the home setting, following physician orders and organization policies.

Requires NJ RN license, BLS, and at least six months RN or one year LPN, preferably in home care or hospice. BSN preferred. We offer comprehensive benefits and competitive pay; the posted starting rate is a minimum base pay with a broad total rewards package.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• At least six months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC experience/skills.
• BSN preferred.

Responsibilities

• Provide professional nursing care to patients in their home following assessment, plan and evaluation.
• Assure compliance with physician orders and documentation reflecting goals at discharge.
• Ensure care follows policy and standard of care; collaborate with the interdisciplinary team.
• Document clinical decisions and coordinate with payors for authorizations as needed.
• Complete nursing assessments, medication reconciliation and update the care plan as patient status changes.
• Teach patients and families; maintain timely communication with management and physicians.
• Supervise home health aides; ensure visits are documented and compliant.
• Maintain awareness of updated standards and uphold organizational competencies.
• Lifts 50 lbs, pushes/pulls 50 lbs and stands up to 8 hours daily.
