Job summary

Hackensack Meridian Health at Home is hiring a registered nurse to provide professional in-home care with a focus on one-to-one patient support. You will assess, plan, implement, and evaluate home nursing needs while ensuring adherence to physician orders and organizational policies.

The role emphasizes compassionate patient education, interdisciplinary collaboration, and documentation accuracy to support optimal outcomes in a home setting.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC Experience/Skills.
• BSN preferred.

Responsibilities

• Provide professional nursing care to patients in their home through assessment, development, implementation and evaluation of home nursing care needs.
• Assure compliance with physician orders and timely visit frequency; document progress toward discharge goals.
• Document care plans, assessments and interventions in the medical chart following policies and standards.
• Coordinate with the interdisciplinary team and coordinate resource referrals and payor authorizations as needed.
• Educate patients and families; implement teaching strategies to meet plan-of-care goals.
• Maintain productivity standards and ensure safe, high-quality care.
