Job summary

Hartford HealthCare at Home is seeking a Registered Nurse to provide skilled nursing care in patients’ homes across Watertown and surrounding towns. You will assess conditions, manage treatment plans, and educate patients and families in recovery and disease prevention.

Qualifications

• Graduate of an Associate Degree or diploma program in nursing.
• Current Registered Nurse license to practice in Connecticut.
• At least one year of nursing experience or one year of experience in a nursing specialty.

Responsibilities

• Provide skilled nursing care to patients in home and community-based settings.
• Assess patient condition, monitor disease progression, evaluate responses to treatment, and carry out the established plan of care.
• Educate patients and caregivers using practical teaching strategies that promote recovery, independence, and disease prevention.
• Coordinate care with physicians, Nurse Care Coordinators, therapists, LPNs, Home Care Aides, and other members of the care team.
• Maintain accurate, timely documentation in accordance with agency standards.
• Connect patients and families with appropriate community resources.
• Direct LPNs and supervise the Home Care Aide plan of care, as appropriate.
• Participate in ongoing education and professional development.
