Hartford HealthCare at Home in Connecticut is seeking a Registered Nurse to deliver high-quality, one-on-one home care to patients in Watertown and nearby communities. You'll assess conditions, implement plans, educate families, and coordinate with the care team.

This full-time position offers comprehensive benefits and professional development within a large, community-focused health system, providing compassionate care in patients' homes across the region.

Formación

• Graduate of an Associate Degree or diploma program in nursing.
• Current Registered Nurse license to practice in Connecticut.
• At least one year of nursing experience or one year of experience in a nursing specialty.
• BSN from an NLN-approved program is preferred.

Responsabilidades

• Provide skilled nursing care to patients in home and community-based settings.
• Assess each patient’s condition, monitor disease progression, eval responses to treatment, and carry out the plan of care.
• Educate patients and caregivers to promote recovery and independence.
• Coordinate care with physicians, care coordinators, therapists, and home care team.
• Maintain accurate documentation and connect families with community resources.
• Supervise Home Care Aide plans of care as appropriate.
