Job summary

Addus HomeCare in Montana is seeking a licensed RN to provide professional home health nursing services according to the patient’s care plan and company policies. The role requires adherence to professional standards and state/federal requirements.

The incumbent will perform nursing procedures, teach patients and caregivers, document findings, coordinate with the care team, and participate in on-call scheduling as needed.

Qualifications

• RN license in the state of practice.
• At least 1 year of clinical nursing experience; home health preferred.
• Ability to perform home visits and document timely.
• Strong interpersonal and written communication skills.
• Reliable transportation and valid driver’s license.

Responsibilities

• Assess, plan and intervene using the nursing process to meet patient needs.
• Observe and report changes in patient condition; document and notify supervisor.
• Perform nursing procedures including administration of treatments and medications per plan.
• Teach patients and caregivers appropriate self-care techniques.
• Notify the Case Manager and physicians of condition changes.
• Maintain supervision visits for Home Health Aide per state regulations.
