Job summary

• Ability to document accurately and timely in clinical records.
• Reliability in meeting schedules and home visit requirements.

Responsibilities

• Deliver professional nursing care per plan of care and policies.
• Assess, plan and intervene using nursing process to meet patient needs.
• Administer treatments and medications per State Nurse Practice Act.
• Teach patients and caregivers self-care techniques.
• Supervise LPN and Home Health Aide visits and evaluate competency.
