Job summary

Shine Bright Care is seeking a Personal Care Assistant to support clients in their homes and communities. You will maintain safety, document progress toward goals, and model professional behavior while building supportive relationships with clients and families.

The role requires following the service plan, communicating with the team, and ensuring accurate notes and compliance with confidentiality regulations.

Qualifications

• Strong interpersonal and customer service skills.
• Knowledge of HIPAA regulations to protect confidential information.
• Experience documenting in Therap database or similar systems.
• Ability to produce clear, accurate notes on interventions and responses.
• Excellent written and verbal communication skills.
• Reliable attendance and punctuality is required.
• Maintain confidentiality of client information at all times.
• Strong organizational skills and ability to follow instructions.

Responsibilities

• Provide ongoing services to clients in home or community settings.
• Maintain safety and document daily progress toward goals.
• Show positive role model behavior and maintain professional boundaries.
• Communicate openly with the care team and document needs.
• Develop individualized service plans with family and team input.
• Coordinate transportation and assist with life domain transitions.
• Ensure timely completion of all documentation per agency standards.
• Attend training and maintain required certifications.
