Job summary

Trinity Health PACE programs in Massachusetts seeks an In-Home Attendant to provide personal care and health-support services to participants at home and in center settings.

You will assist with daily living tasks, mobility, meals, and safety, while documenting care, attending meetings, and collaborating with a caregiving team; CNA preferred; own insured vehicle often required.

Qualifications

Shift: Description: Full Time - M-F + every other weekend (1 week day OFF when your ON the weekend)
Pay Range: $17.11-$20.10/hr
Sign On Bonus: $750

Position Purpose

The In-Home Attendant is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants.

What you will do
  • As part of the health care team, provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming.
  • Assist in the maintenance of physical activity, exercise, and walking.
  • Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the center.
  • Responsible for participant safety, identify safety hazards and emergency situations, and initiate corrective action as able.
  • Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance.
  • Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
  • Assist in identifying signs and symptoms indicating physical, emotional, or behavioural change in participants and reporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.
  • Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards.
  • Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.
  • Assist transportation staff with loading and unloading of participants from vehicles.
  • Assist participants being transported in PACE Organization vehicles as needed.
  • Attend staff meetings and participant care planning on a regular basis.
  • Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participants expressed wishes.
  • Prepare and submit timely written reports as required.
  • Participant assistance may be required in the center, clinic, or home health/In Home Services department.
  • Attend and participate in staff meetings, in-services, projects, and committees as assigned.
  • Maintain strict and confidentiality of all participant information
Minimum Qualifications
  • High school diploma or GED required.
  • Active certified nurse assistant in the state where PACE Organization is located preferred but not required.
  • Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
  • Experience in a long-term care facility or with a community-based geriatric program preferred.
  • Own insured vehicle for transportation.
  • Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting.
  • Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
  • Physical mobility that includes movement from place to place.
  • Must be able to sit for long periods of time.
  • Physical agility, that includes ability to maneuver body while in place.
  • Dexterity of hands and fingers.
  • Coordination, including eye-hand, hand-foot.
