Job summary

Trinity Health PACE seeks an In-Home Attendant to provide compassionate personal care to participants in home and center settings. You will assist with daily activities, meals, mobility, and safety while supporting nursing care tasks as assigned.

The role emphasizes infection control, documentation, and collaboration with a health care team. A CNA and elder-care experience are preferred, with a strong commitment to patient-centered care.

Qualifications

• High school diploma or GED required. Active CNA license preferred.
• At least 1 year of experience with frail or elderly populations.
• Experience in long-term care or geriatric programs preferred.

Responsibilities

• Provide personal care assistance to maintain independence in activities of daily living (ADLs) such as toileting, bathing, eating, grooming, and mobility.
• Provide nursing care as assigned following center policies.
• Ensure participant safety, identify hazards, and take corrective actions as needed.
• Offer behavioral support and reassurance, support medication instructions and dietary needs.
• Assist with activities, meals, recreation, and light housekeeping.
• Observe health changes and report to supervisors; document changes and care provided.
• Maintain infection control and safety standards; assist with transportation and appointments.

The In-Home Attendant is responsible for providing various care services necessary for attending to the personal needs, support, and health status of participants.

What You Will Do

• As part of the health care team, provide personal care assistance that will maintain the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming. Assist in the maintenance of physical activity, exercise, and walking.
• Provide nursing care as assigned in a safe manner, consistent with the policies and procedures of the center.
• Responsible for participant safety, identify safety hazards and emergency situations, and initiate corrective action as able.
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance.
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
• Assist in identifying signs and symptoms indicating physical, emotional, or behavioral change in participants and reporting these signs to the center staff and/or supervisor, including but not limited to, any changes in skin integrity, bowel and bladder status, food/fluid intake, or changes in ambulation.
• Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards.
• Escort participants to medical, dental, optometry, rehabilitation, and other appointments as necessary.
• Assist transportation staff with loading and unloading of participants from vehicles. Assist participants being transported in PACE Organization vehicles as needed.
• Attend staff meetings and participant care planning on a regular basis.
• Maintain current written records, indicating ongoing documentation of services provided, reassessments of changing needs, and participants expressed wishes.
• Prepare and submit timely written reports as required. Participant assistance may be required in the center, clinic, or home health/In Home Services department.
• Attend and participate in staff meetings, in-services, projects, and committees as assigned.
• Maintain strict and confidentiality of all participant information

Minimum Qualifications

• High school diploma or GED required. Active certified nurse assistant in the state where PACE Organization is located preferred.
• Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
• Experience in a long-term care facility or with a community-based geriatric program preferred.
• Primarily works inside with frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
• Physical mobility that includes movement from place to place.
• Must be able to sit for long periods of time.
• Physical agility, that includes ability to maneuver body while in place.
• Dexterity of hands and fingers.
• Coordination, including eye-hand, hand-foot.

Pay Range: $17.11-$20.10/hr
