Job summary

National Recruiters is actively seeking Registered Nurses for in-home health assessments across New Jersey. Travel to patients' homes to perform comprehensive health assessments is required, with a flexible schedule and immediate openings for independent nurses.

As an In-Home Assessment RN, you will review medical history, reconcile medications, document vitals, and communicate findings to the care team. Home health or primary care experience is a plus.

Qualifications

• Active, unrestricted New Jersey RN license.
• Minimum of 3 years of clinical experience.
• Strong patient assessment and clinical judgment.
• Ability to work independently in patients' homes.
• Excellent communication and interpersonal skills.
• Strong documentation and EMR skills.
• Willingness to travel within territory.
• Availability to commit to at least 50 hours per month.

Responsibilities

• Reviewing medical history and current health conditions
• Completing medication reconciliation
• Performing a review of systems
• Obtaining and documenting vital signs
• Conducting a comprehensive physical assessment
• Identifying and documenting current and historical diagnoses
• Performing appropriate point-of-care testing when required
• Using your clinical judgment to identify potential health concerns
• Accurately documenting assessment findings in an electronic medical record
• Communicating professionally and compassionately with patients
