Job summary

Cooper University Health Care in South Jersey seeks an Advanced Practice Provider (APP) – Per Diem – to deliver in-home longitudinal primary care to adults and geriatric patients. The APP will work autonomously or with a supervising physician, performing history, exams, treatment planning, and prescribing medications per state and APP guidelines, with emphasis on preventive care and patient education.

Experience with primary care, chronic care management, and home-based care is preferred.

Qualifications

• Minimum 3 years’ experience serving adults and geriatrics as an APP.
• Specialization in Primary Care and Chronic Care Management is preferred.
• NJ APN/NJ RN or NJ PA licensure required.

Responsibilities

• Provide in-home longitudinal primary care to adults.
• Perform and manage patient history, exams, and care plans.
• Prescribe medications per APP guidelines and state laws.
• Collaborate with supervising physician and other health professionals.
