Qualifications

• Active California Speech-Language Pathologist license required.
• CCC certification preferred.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Evaluate, diagnose, and treat speech, language, communication, and swallowing disorders.
• Develop and implement individualized treatment plans for residents.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Monitor and document resident progress and adjust treatment plans as needed.
• Educate residents, families, and staff on therapy techniques and care plans.
• Maintain accurate and timely documentation in accordance with facility and regulatory guidelines.
• Ensure compliance with all safety, infection control, and HIPAA standards.
• Participate in care planning meetings and required in-service trainings.
