Job summary

Glengariff Rehabilitation and Healthcare Center seeks a Speech Language Pathologist (SLP) to assess patients, develop and administer rehabilitation programs, and set goals with input from the physician. You will document patient conditions and educate families while collaborating with an interdisciplinary team.

The role emphasizes care for sub-acute and long-term residents, strict confidentiality, HIPAA compliance, and timely scheduling.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for NY.
• Experience with long-term care and/or geriatric care is preferred but not required.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Punctuality and adherence to scheduled hours; professional demeanor.
• Perform all other duties assigned by Director of Rehabilitation.
• Compliance with confidentiality, HIPAA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document and record patient condition; educate patients and families.
• Collaborate with an interdisciplinary team to improve life and function.
• Maintain confidentiality and adhere to HIPAA and Residents' Rights.
