Job summary

The Northern Metropolitan Center is seeking a dedicated Social Worker to join our team. You will provide emotional, social, and psychological support to patients and residents while conducting social histories, developing care plans, and coordinating with healthcare providers.

You will participate in interdisciplinary teams, maintain accurate records, connect clients with community resources, and contribute to ongoing quality improvement.

Qualifications

• Minimum 0 years of experience in social work or related field.
• Bachelor's degree in Social Work or related field is required.
• Current licensure as a Social Worker (LSW, LCSW, etc.)

Responsibilities

• Conduct social histories and assessments to identify patient/resident needs and develop personalized care plans.
• Provide counseling, education, and support to patients, residents, and families.
• Coordinate with healthcare providers and other professionals to ensure comprehensive care.
• Facilitate connections with community resources and services.
