Job summary

WyndRidge Health and Rehabilitation Center in Crossville, TN is seeking a Social Worker to promote residents' psychosocial well-being by providing emotional support, care planning, and advocacy. This role collaborates with residents, families, physicians, and the interdisciplinary team to ensure person-centered care while complying with regulations.

Qualifications

• Bachelor's Degree in Social Work (BSW) and active state license.
• Experience in skilled nursing or long-term care preferred.
• Knowledge of CMS regulations, discharge planning, and long-term care standards.
• CPR certification preferred.

Responsibilities

• Conduct psychosocial assessments on admission and as needed throughout the resident's stay.
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
• Provide emotional support, counseling, and crisis intervention to residents and families.
• Coordinate discharge planning to ensure safe transitions to home or appropriate care levels.
• Advocate for resident rights and ensure dignity, independence, and quality of life.
• Maintain accurate, timely documentation in line with policies and regulations.
• Ensure compliance with CMS, state, and federal regulations related to social services.
• Participate in QAPI initiatives.
• Build relationships with community resources to support resident needs.
