• Gathers data for court petitions and dispositional review hearings.
• Maintains contact via phone, home visits, or site visits with involved agencies.
• Implements interventions and monitors progress toward case plan goals.
• Maintains detailed documentation of client participation and required forms.
• Assesses safety and risk and develops safety plans as needed and coordinates with parties.
• Uses a person-centered approach to determine service needs and referrals.
