Responsibilities

• Advocate for residents' psychosocial, emotional, and social needs.
• Complete social services assessments and participate in care plan development.
• Meet with residents and families to identify concerns and provide support.
• Coordinate and participate in interdisciplinary team meetings and care conferences.
• Assist with discharge planning and transition to/from Skilled Nursing.
• Maintain timely documentation in accordance with facility policies.
