Job summary

The Pennant Group, Inc. is seeking a Social Worker Assistant to implement standards of care for social work services under the supervision of a Licensed Medical Social Worker.

The role involves assessing patients’ psychosocial status, conducting social evaluations, and maintaining clinical records in a home health setting. Qualifications

• Must have a bachelor’s degree in social work, psychology, sociology, or related field.
• Minimum of one year social work experience in health care setting.
• CPR certification is required.
• Must be a licensed driver with a vehicle.
• Experience in home health care is preferred.

Responsibilities

• Assesses psychosocial status and communicates findings to nurse.
• Carries out social evaluations and plans interventions.
• Maintains clinical records for social work referrals.
• Provides information and referrals to patients, families, and community agencies.
• Liaises between patients/families and community services.
• Participates in discharge planning with the Licensed Medical Social Worker.
• Supports development of the overall care plan and case conferences.
• Adheres to documentation standards and professional conduct.
