Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Speech Language Pathologist (SLP) to evaluate and treat residents with speech, language, voice, and swallowing disorders. The role offers full-time or part-time opportunities on a collaborative rehabilitation team.

As an SLP, you will develop and implement individualized treatment plans, monitor progress, and coordinate with physicians and nursing staff.

Qualifications

• Active Florida Speech Language Pathologist license required.
• CCC preferred.
• Experience in skilled nursing or rehab setting preferred.
• Strong assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work within an interdisciplinary team.

Responsibilities

• Evaluate residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on resident needs and goals.
• Provide speech therapy services to improve communication, cognition, and swallowing function.
• Monitor resident progress and adjust treatment plans as appropriate.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support positive resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards.
• Educate residents and families regarding therapy programs and treatment goals.
• Participate in care planning meetings and discharge planning processes.
• Ensure compliance with all federal, state, and facility policies and procedures.
• Maintain a safe, organized, and supportive therapy environment.
