Job summary

Flagstar Rehab is seeking a Speech Language Pathologist (SLP) to join our skilled nursing facility teams in NYC and surrounding areas. The role focuses on comprehensive evaluations, treatment planning, and patient/family education to enhance speech and swallow function.

We offer direct employment or independent contracting with setup assistance for incorporation. Pay rates are $46–$48 per hour (W-2) or $57–$60 per hour (1099).

Qualifications

• Current NYS SLP License and Registration
• Clinical Competence Certification (CCC)
• CPR Certificate
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility

Responsibilities

• Provide speech and swallow evaluations and treatments in SNF settings.
• Develop and manage plans of care, adjust as needed, manage textures.
• Deliver therapy to patients, educate families, and document progress.
