Job summary

Relient Health is seeking a full-time Speech Language Pathologist (SLP-CCC) for a Skilled Nursing Facility (SNF) in Chelmsford, MA. You will evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders, while collaborating with a rehab team in a supportive environment.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current MA SLP license (or eligibility).
• CCC-SLP certification required for this location.
• Strong interpersonal and documentation skills.

Responsibilities

• Evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders.
• Develop and implement individualized treatment plans.
• Document patient progress using EMR systems.
• Collaborate with PTs, OTs, nurses, and physicians to deliver coordinated care.
• Educate patients, families, and staff on treatment goals and progress.
