Job summary

Beth Israel Lahey Health, Beverly Hospital is seeking a Case Manager RN for a Per Diem Day shift in Beverly, MA. The role coordinates care, conducts concurrent and retrospective reviews to ensure appropriate utilization, and facilitates discharge planning with patients, families, insurers, and care agencies.

Candidates should be a graduate of an accredited nursing program with MA RN licensure and 2–3 years of case management experience; BSN preferred and case management certification preferred.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• 2–3 years of Case Management experience preferred.
• Current Massachusetts RN licensure; Certification in Case Management preferred.

Responsibilities

• Coordinate care as the care coordinator for patients, caregivers, and families.
• Perform concurrent and retrospective reviews to ensure appropriate utilization and prevent denials.
• Coordinate safe discharge planning with the health care team, patients, families, insurers, and continuing care agencies.
