MEDEX STAFFING NETWORK LLC is seeking an experienced Case Manager RN for a 12-week contract in Los Angeles, CA. The role runs Monday through Friday, 8:00 am to 4:30 pm, offering meaningful patient care opportunities within a healthcare staffing network.

The ideal candidate has CA RN licensure and at least 2 years of acute care case management or utilization review experience, with familiarity with InterQual or Milliman guidelines and strong EHR skills. CCM certification is a plus.

Qualifications

• Active RN license in the state of California (required).
• Minimum 2+ years of case management or utilization review experience in an acute care setting.
• CCM certification is a plus.
• Strong knowledge of InterQual or Milliman care guidelines preferred.
• Proficiency with electronic health record (EHR) systems.
• Excellent communication, critical thinking, and organizational skills.

Responsibilities

• Coordinate and manage patient care plans across the continuum of care to ensure optimal outcomes.
• Conduct utilization reviews and collaborate with interdisciplinary care teams to facilitate appropriate level of care.
• Assess patient needs, develop individualized care plans, and monitor progress throughout the care episode.
• Facilitate timely discharge planning and coordinate post-acute care transitions and community resources.
• Communicate effectively with physicians, payers, patients, and families regarding care coordination and discharge goals.
• Document case management activities accurately in the electronic health record (EHR) system.
• Advocate for patients to ensure access to appropriate healthcare services and resources.
