Job summary

Cano Health is seeking a Risk Adjustment Coder to identify and document CMS HCC related diagnoses based on patient encounters. You will review medical records, prepare charts, and ensure coding accuracy with Excel support and coordination with care teams.

Ideal candidates have CPC/CPC-A/CCS-P/CRC certification, 2+ years of medical coding experience, and strong attention to detail. Travel to offices may be required to conduct on-site chart reviews and support quality initiatives.

Qualifications

• Two (2) years of medical coding experience.
• Experience with CMS HCC categorization and risk adjustment.
• Proficient in Excel for tracking and reporting.
• Strong written and verbal communication.

Responsibilities

• Review records to identify CMS HCC codes.
• Prepare charts and track data in Excel.
• Document claim/encounter information accurately.
• Provide coding guidance to clinical staff.
• Support MRA/HEDIS/STARS coding with care teams.
• Monitor CMS coding changes for updates.
• Process HCC suspect reports and on-site reviews.
• Code and submit encounters promptly.
• Resolve coding questions for providers.
• Update leadership on weekly status.
• Notify when annual wellness visits are not scheduled.
• Travel to offices for chart reviews.
• Participate in process and quality improvements.
• Assist with billing claims as assigned.
