Job summary

Monroe Health & Rehab Center is seeking a Physical Therapist to help residents in a skilled nursing facility regain functional independence. You will design and implement therapy plans, collaborate with physicians and the therapy team, and guide patients through activity modification and home exercise programs.

Responsibilities include teaching patients how to maintain gains after discharge, using EHR systems, and delivering care in a supportive, team-driven environment.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited college or university.
• Active, unencumbered Physical Therapy license for the state of employment.
• Basic computer skills and ability to navigate service delivery software and EHR systems.

Responsibilities

• Restore patients to optimal physical function within a SNF setting and work with therapy goals with patients, physicians, and staff.
• Educate patients on maintaining function after discharge through independent exercise and activity modification.
