The role requires a Master’s or Bachelor's degree in Physical Therapy, an active license (or eligibility) in the state, and proven competency in assessment, planning, and documentation.

Qualifications

• Master of Science in Physical Therapy or Bachelor of Science in Physical Therapy with 5+ years of direct practice.
• Current license or eligibility for license in the practice state.
• Demonstrates competency in applying PT principles, methods, and equipment.

Responsibilities

• Review patient information and assess impact on treatment.
• Perform thorough functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Update treatment plans as needed during care.
• Complete documentation per laws and standards.
• Supervise therapy assistants as allowed by state guidelines.
• Educate patients and families on procedures and risks.
• Communicate with nursing staff and families to ensure care delivery.
