Reliant at Home is actively seeking a PRN Occupational Therapist for Johnson County and surrounding areas to provide home health services. Home health experience is strongly preferred, with a commitment to high-quality patient care and community involvement.

Qualifikationen

• Home health clinical experience preferred.
• Assess and address patient needs through occupational therapy.
• Meet mandatory continuing education requirements of the agency/licensing board.

Aufgaben

• Perform clinical assessments to identify patient needs and appropriateness of care.
• Document, prepare/revise and complete a Plan of Care, progress notes, and other clinical records.
• Apply infection control and standard precautions in coordinating/performing patient care activities.
• Assess and facilitate ROM, strength, coordination, adaptive equipment, functional mobility and safety for ADL performance; fabricate adaptive equipment as needed.
• Use effective interpersonal relations and communication to coordinate with the team.
• Evaluate patient occupational performance in self-care, work, leisure and rest.
• Assess sensory-motor, psycho-social and cognitive-perceptual components of occupational behavior and environmental factors.
• Meet mandatory continuing education requirements of the agency/licensing board.
• Demonstrate commitment, professional growth and competency.
• Promote agency philosophy and administrative policies to ensure quality of care.
