Job summary

South Coast Post Acute in Santa Ana, CA is seeking a Primary Counselor to provide therapeutic support, assess client needs, conduct individual and group counseling, and implement personalized behavioral plans.

You will work with an interdisciplinary team under the Program Director, maintain case notes, monitor progress, and help develop treatment plans to address stress, mental health, and life challenges. Two years of direct care in behavioral settings or related education is required.

Qualifications

• Two years direct care work experience in an inpatient or outpatient behavioral setting or two years of college-level coursework including behavioral health or a combination of relevant experience and education totaling two years.

Responsibilities

• Provide individual and group counseling sessions and implement individualized behavioral programs.
• Assess client needs and maintain case notes; monitor progress and contribute to treatment plan development.
• Collaborate with an interdisciplinary team under the supervision of the Program Director to support clients’ mental health and life challenges.
