Job summary

Northshore Home Health Care is seeking a compassionate physical therapist to provide services for patients with musculoskeletal, neurological, and functional limitations in inpatient and outpatient settings. You will evaluate functional status, develop evidence-based treatment plans, and educate patients and families on diagnoses and home care.

You will collaborate with an interdisciplinary care team to coordinate goals and ensure high-quality outcomes, while continuously supporting quality care

Qualifications

• Licensed physical therapist with musculoskeletal and neurological rehabilitation expertise.
• Strong clinical judgment, comprehensive assessments, and individualized care plan development.
• Excellent communication and patient-centered education skills.
• Ability to work effectively in diverse, collaborative healthcare environments.

Responsibilities
