Qualifications

• DPT degree or accredited physical therapy degree is required
• Active state licensure as a Physical Therapist is required
• BLS certification is required
• Clinical experience in physical therapy is preferred

Responsibilities

• Perform initial evaluations and re-evaluations with individualized plans of care
• Deliver therapeutic exercise, manual therapy, gait training and modalities
• Set measurable goals and track patient progress
• Supervise physical therapist assistants and aides as appropriate
• Educate patients and caregivers on home programs and safety
• Document evaluations, treatments and discharge planning accurately
