Job summary

• Must hold or be eligible for State of Michigan physical therapy licensure within 30 days of applying.
• Bachelor degree in Physical Therapy with 5 years of experience OR Master’s/Doctorate in Physical Therapy from a CAPTE-accredited program.

Responsibilities

• Performs patient assessment, treatment, discharge planning and collaboration with other health care providers.
• Documents patient care and billing information timely and in accordance with department standards.
• Communicates with co-workers and other professionals to ensure optimal patient care.
• Acts as a team member in care and mentoring with staff.
• Maintains clinical productivity standards and ongoing clinical education.
• Helps ensure patient safety and maintains confidentiality per HFHS policies.
