Job summary

Lifepoint Rehabilitation is seeking a Physical Therapist (PT) to join our team at Conway Medical Center. You will assess patients, develop and implement treatment plans, and educate patients on home exercises to improve strength, mobility and function.

As part of Lifepoint Health, you will document outcomes, collaborate with rehab teams, and participate in discharge planning to support community health. A current PT license in NC is required.

Qualifications

• Education: Graduate of an accredited physical therapy program.
• Previous rehab experience preferred.
• Certifications: BLS.
• Licenses: Current Physical Therapy license in NC.

Responsibilities

• Conduct physical therapy assessments and develop treatment plans.
• Provide direct patient care to restore strength, mobility, and function.
• Educate patients and caregivers on home exercise programs.
• Document treatment sessions and patient outcomes accurately.
• Collaborate with rehab teams and participate in discharge planning.
• Perform other duties as assigned.
