Mid-State Health Center in Plymouth, NH is seeking a full-time Physical Therapist to work directly with patients in an outpatient setting. You will collaborate with the medical team to develop and adjust treatment plans, perform assessments, and document progress under a physician’s plan of care.

The ideal candidate holds an APTA-accredited degree, NH license, and has 5+ years of PT experience, including primary health care exposure.

Qualifications

• Graduate of an APTA-accredited program and NH PT license.
• Bachelor's degree required; Doctorate preferred.
• Current NH State PT license.
• Minimum of 5 years PT experience.
• 1–3 years in primary health care setting preferred.

Responsabilités

• Assess patients and document per physician plan.
• Develop and modify rehabilitation plans.
• Perform range of motion, sensory and muscle tests.
• Conduct gait analysis and functional assessments.
• Instruct patients and families on rehab protocols.
• Coordinate referrals to community resources as needed.
• Maintain knowledge of licensure and continuing education.
• Ensure safety and respond to emergencies.
• Meet productivity and documentation guidelines.
• Apply evidence-based chronic illness management where appropriate.
• Collaborate with clinical team for patient care.
