Responsibilities include outreach in home and community settings, crisis prevention plans, developing recovery goals, and coordinating with community resources. The role emphasizes trauma-informed, strength-based support and requires travel to client homes and programs.

Qualifikationen

• CRPA or CPS certification required; provisional considered if in process of obtaining permanent certification.
• Minimum two years human services experience preferred, including experience working with young adults.
• Strong interpersonal, communication, advocacy, and relationship-building skills.

Aufgaben

• Engage young adults in a respectful, strength-based, culturally competent manner and build trusting relationships.
• Assist clients in identifying strengths, needs, goals, and barriers to independent living, education, employment, and recovery.
• Provide peer mentoring, advocacy, and wellness coaching to promote self-sufficiency and community integration.
• Support clients in developing daily living skills, including budgeting, medication management, personal wellness, and transportation.
• Assist clients in accessing community resources and services, including behavioral health, housing, and vocational programs.
• Conduct home, community, and office visits, and participate in psychoeducational and skill-building groups.
