Job summary

Home Base, Inc. is seeking an Outreach Counselor in Weirton, WV to support at-risk families. The role involves direct service, documenting progress, and coordinating with DHHR case managers. Training in Nurturing Parenting will be provided to certifiy you in a nationally recognized program.

The position starts part-time with the potential to become full-time and includes a comprehensive benefits package for eligible employees, such as health/dental/vision insurance and tuition reimbursement.

Qualifications

• Bachelor’s Degree in Social Work or Human Services related area required.
• Master’s Degree in Social Work and licenses preferred, but not required.
• Must have valid Driver’s License and Insurance; ability to operate a vehicle in state of residence.

Responsibilities

• Establishing personalized service plans for clients and providing systematic interventions.
• Opening cases within mandated time frames and scheduling visits weekly.
• Providing services within the AHG Guidelines and NASW/ASO codes of ethics.
• Documenting all contacts with clients and reporting to DHHR case managers.
• Maintaining orderly client records and participating in team meetings.
• Testifying in court regarding client progress and case closure.
