Qualifications

• The PT must assess and plan rehabilitation programs with patients and families.
• Demonstrates empathy, professionalism and effective communication with the care team.
• Maintains licensure, complies with hospital and regulatory standards.

Responsibilities

• Implements the physical therapy treatment plan and selects activities to progress goals.
• Documents evaluation and plan of care according to guidelines.
• Provides ongoing patient education, discharge instructions and follow-up as needed.
• Acts as a resource for staff and families regarding rehabilitation services.
