BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time opportunity providing outpatient visits in the Annandale, VA area at our senior living office. The role emphasizes independent, patient-centered care and collaboration within a multidisciplinary team.

Qualified candidates hold a VA PT license and an APTA/AMA-accredited program credential, with strong communication and EMR skills.

Qualifications

• VA PT license.
• PT program approved by APTA or AMA accreditation.
• Ability to work independently.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation, strength and endurance, balance testing, and safety assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan, and coordinate with nursing in the multidisciplinary plan.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
