BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for PRN, Part-Time, or Full-Time home health visits in the Annandale, VA area. This role supports adult and geriatric clients in their homes as part of a multidisciplinary team.

You will perform diagnostic tests, develop care plans, educate clients and families, and document care using an EMR. Strong communication and organizational skills are essential to ensure safe, effective therapy and patient independence.

Qualifications

• Current PT license (VA)
• Graduate from an APTA/CAAHEA-accredited program
• Ability to work independently and manage time
• Strong interpersonal, organizational, and problem-solving skills
• Solid computer skills; EMR experience preferred

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, strength, balance).
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and implement individualized care plans including manual therapy, gait training, and balance.
• Continuously assess and revise the PT care plan with nursing and the multidisciplinary team.
• Educate clients and family members in rehabilitative care for health and independence.
• Document observations and care in the EMR with accuracy.
