Job summary

• Conduct evidence-based evaluations to determine patients' functional abilities and goals.
• Deliver therapeutic interventions to improve independence in activities of daily living.
• Develop individualized care plans in collaboration with the patient and care team.
• Educate patients and families on adaptive techniques and equipment.
• Collaborate with physicians, nurses, PT/SLP, and social work to coordinate care and discharge planning.
• Document in the EMR accurately and timely.
• Support quality, safety, and performance improvement initiatives across the MedStar system.
