Responsibilities

• Assess patients in collaboration with physicians to establish and evaluate treatment programs.
• Develop plan of care with patient and family that is realistic and discharge-focused.
• Provide ongoing education and training to patients and families.
• Implement OT treatment plans and select activities to progress toward goals.
• Document evaluations, plans of care, and treatment sessions per guidelines.
• Coordinate discharge planning and referrals to community resources as appropriate.
• Maintain confidentiality and adhere to ethical standards and infection prevention precautions.
• Engage in staff meetings, orientation, and mandatory competencies.
