Job summary

CMU Medical Education Partners is seeking an experienced Social Worker for the OB/GYN department to coordinate care and behavioral health interventions for patients with complex needs.

The role collaborates with OB/GYN, Family Medicine, and Pediatric teams to ensure cost-effective, quality outcomes and effective patient advocacy. Requires MSW and Michigan LMSW, CPR certification, and 3+ years of relevant experience.

Qualifications

• Master's degree in social work from an accredited school is required.
• Current LMSW license in Michigan is required.
• CPR certification is required (BLS/Heart Association acceptable).
• Minimum three years of social work experience; five years preferred.
• Certification in perinatal mental health within 6 months of hire.
• General computer knowledge and ability to use computers.

Responsibilities

• Plans, coordinates, monitors, and evaluates all options to optimize patient health.
• Manages caseload commensurate with experience and complexity.
• Advocates for patients, coordinates care across continuum, educates families.
• Performs transition of care calls within two business days of discharge.
• Works with CMEP leadership to develop social service programs and policies.
• Maintains documentation and codes for services provided.
• Maintains records and progress notes per policy.
• Builds relationships with community resources and represents CMU Health.
• Occasionally supervises social work students and provides education to the team.
• Acts as resource on social and emotional factors influencing health problems.
• Keeps knowledge of Medicare/Medicaid and third-party reimbursement.
• Participates in quality improvement activities.
• Delivers positive customer relations with patients, families, physicians, and staff.
• Adheres to department/hospital policies.
