Job summary

Apex Nursing and Rehabilitation Center is seeking a dedicated Long Term Care Dietitian to provide nutrition care to residents, develop individualized plans, and collaborate with the care team to support wellness.

This role requires a Bachelor's in Dietetics or Nutrition, an RDN credential, at least 1 year of clinical nutrition experience (prefer LTC), and strong knowledge of therapeutic diets and regulatory standards.

Qualifications

• Bachelor’s degree in Dietetics, Nutrition, or related field.
• RDN credential required.
• Minimum 1 year of clinical nutrition experience, preferably in long-term care.

Responsibilities

• Conduct nutritional assessments and develop care plans for residents.
• Plan and review menus to meet dietary needs and regulatory standards.
• Educate residents and families on nutrition and dietary options.
• Coordinate care with physicians, nurses, and other healthcare professionals.
• Ensure compliance with state and federal regulations and maintain records.
• Participate in quality improvement initiatives related to dietary services.
• Provide training and guidance to dietary staff.
