Job summary

Rocky Mountain Care Group is seeking a dedicated LPN for our Independence facility in Missouri. The role involves delivering nursing care to a small group of residents, performing assessments, and coordinating care plans with CNAs and the nursing team.

You will handle admissions, discharges, IVs, wound care, and medication administration in a fast-paced rehab setting. A supportive nursing leadership presence is available to assist as needed.

Qualifications

• Current LPN license to practice in Missouri.
• Ability to assess residents and develop care plans.
• Ability to make independent decisions when warranted.
• Knowledge of nursing practices, laws, and long-term care guidelines.
• Strong interpersonal skills and team collaboration.
• Basic computer skills including email.
• Proven written and oral communication skills.
• Ability to relate well with residents, families, and staff.

Responsibilities

• Deliver nursing care to residents in a nursing unit.
• Assess resident needs and develop individualized care plans.
• Supervise CNAs and other staff in delivering care.
• Administer medications and monitor resident status.
• Collaborate with an interdisciplinary care team to achieve outcomes.
