Collage Home and Community Rehabilitation Partners in Sugar Land, TX seeks experienced Licensed Clinical Social Workers for part-time positions. Clinicians will provide patient-centered care in patients' homes and communities, assessing and treating individuals with neurological and complex orthopedic conditions.

Qualificações

• Completion of a Master’s degree in Social Work and state licensure as an LCSW are required
• Excellent planning, organization, writing, and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must

Responsabilidades

• Assess, counsel, and direct medically complex patients by developing individualized treatment plans to aid adjustment, resources, and behavior modification
