Job summary

Guardian Home Health is seeking a Speech Therapist to deliver home health services, assessing and treating speech, language, swallowing, and cognitive impairments and developing treatment plans. You will monitor progress and adapt interventions to improve patient well-being in their home settings.

Requirements include licensure in Speech Therapy in the state of residence and a valid driver’s license with reliable transportation. CPR is preferred; weekend rotations may be available.

Qualifications

• Licensed in Speech Therapy in the state of residence.
• Current driver’s license with access to transportation.

Responsibilities

• Provides services within the scope of practice as defined by state laws and the plan of care.
• Evaluates function and assists in developing the therapy plan of care.
• Treats patients to improve communication according to condition.
• Observes and reports patient responses to treatment and changes in condition.
• Instructs patient, family and care team in speech therapy activities.
• Functionally supports patients across age ranges and backgrounds.
