Qualifications

• Master's degree in Social Work required.
• Tennessee Licensure as a Master Social Worker required; Tennessee L.C.S.W. preferred.
• Valid TN Driver’s License required.
• Minimum of 1 year post‑Master's clinical experience preferred.

Responsibilities

• Assess patient's/family's biopsychosocial functioning, emotional support network and socioeconomic status and needs.
• Develop individualized and attainable goals in conjunction with patients and families.
• Provide counseling for adaptation to medical circumstances and psychosocial effects.
• Discharge planning integrates medical and psychosocial plans of care; knowledge of community resources.
