Job summary

Optimal Care in Jackson, MI is seeking a Home Health Speech-Language Pathologist (SLP) to assess and treat communication and swallowing disorders in patients' homes. You will deliver evidence-based therapy, develop individualized plans, and collaborate with physicians and the care team to support functional goals.

You will conduct comprehensive evaluations, educate patients and caregivers, maintain compliant documentation, and travel within the service area to provide care across settings.

Qualifications

• Master’s or Doctoral degree in Speech Language Pathology.
• State Licensed as a Speech Language Pathologist.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

• Perform comprehensive evaluations and treat speech, language, cognitive, and swallowing disorders.
