Job summary

Optimal Care is seeking a Home Health Speech-Language Pathologist to evaluate and treat speech, language, cognitive, and swallowing disorders in patients’ homes. You will develop individualized plans of care, collaborate with physicians and the care team, educate patients and caregivers, and ensure compliant documentation.

You will travel within Macomb/Oakland and Wayne counties, with daily in-home visits, Monday–Friday, 8 a.m.

Qualifications

• Master's or Doctoral degree in Speech-Language Pathology.
• State Licensure as a Speech-Language Pathologist.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.

Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans.
• Deliver evidence-based therapy to help patients regain communication and swallowing abilities.
• Collaborate with physicians and interdisciplinary care team; educate patients and caregivers.
• Maintain accurate clinical documentation that meets regulatory and reimbursement requirements.
