AccentCare Inc is seeking an experienced RN Case Manager for home health serving Lancaster County, PA and surrounding areas. This full-time role manages patient care plans from admission through discharge, coordinating direct and indirect care and guiding LPNs, CHHAs, and other staff to deliver the plan of care.

Compensation is $87,000 to $100,000 per year, paid on a per-point basis, with annualized estimates.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as an RN in PA.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess the status of nursing needs and manage the patient’s care plan.
• Complete initial patient assessment upon admission, including history.
• Select appropriate nursing diagnosis.
• Assess medicines for adverse reactions or contraindications.
• Establish goals aligned with the diagnosis.
• Inform the physician of assessment results and interventions.
• Implement nursing interventions within the Nurse Practice Act.
• Re-evaluate patients’ needs as required.
• Document assessments, progress, and discharge notes timely.
